Antony Stewart was the main author. He designed the protocol and piloted and modified the forms used for assessment of eligibility, validity and data extraction. He undertook searches for studies and requested further information from authors. He liaised with content experts in the field to obtain background information and support. He assessed studies for eligibility, validity and extracted data from them. He collated the data and wrote the report. Amanda Burls was the main editor. She advised on the protocol for the review.
